Parents/guardians must complete the Minor Donor eConsent for all 16 & 17 year old students

Drink plenty of water, eat a good meal and bring your I.D.

Steps to Complete a Minor Donor eConsent

1. To get started, scan the QR code or visit vitalant.org/econsent
2. Read the form, complete the fields and review responses. Then select "Submit".
3. A receipt with a barcode will be sent to the email addresses provided.
4. On the day of the donation, the barcode can be presented or Vitalant staff can look it up.
